Going Places is an appointment based transportation
service
for
West Deerfield Township residents who have mobility
challenges that may prevent or restrict their ability to
meet
their health care needs.

Who is eligible?
Mobility-challenged residents from West Deerfield
Township must pre-register with the Township
Transportation Department. Due to restrictions, minors
and non-registered passengers are not allowed to ride.
Riders must also complete a form, signed by a physician,
indicating the rider is stable and able to ride in a
sitting position.
Where does the van go?
Trained drivers will pick up riders and deliver them to
appointments at Lake Forest, Condell, Glenbrook and
Highland Park Hospitals as well as the V.A. Hospital in
North Chicago. Other locations must be within a five
mile radius of the Township. A modified mini-van
equipped with a ramp will accommodate up to two
wheelchairs and three additional riders.
When is the service available and how does it work??
Appointments must be made no earlier than 9:00 a.m. and
finish no later than 3:30 p.m. Pick-up times are
determined by the rider's appointment time. When
finished with an appointment, riders will call the
driver for a return ride. The wait for a return ride can
be up to 15 minutes.
Passengers should be ready 15 minutes prior to any
scheduled pick-up time in order to ensure they are ready
when the driver arrives. Drivers are only allowed to
wait five minutes. Please remember that last minute
changes and cancellations disrupt the service operations
for everyone. Generally, the rider is taken directly to
the provider location with no other stops.
How do I make an appointment?
Rides may be scheduled directly with the Transportation
Secretary between 12:00 p.m.-3:00 p.m. Monday through
Friday. At other times, you can leave a message and the
secretary will call you back. Medical appointments may
be made up to 30 days in advance. If scheduling allows,
non-medical trips for eligible riders may be made three
business days in advance. Reservations for medical
appointments will have priority. Riders are eligible for
up to two round trips weekly.
What are my responsibilities as a passenger?
It
is the responsibility of the passenger to ensure their
personal equipment and/or aids are in acceptable working
order. If a wheelchair or scooter is used, a ramp must
be installed at the passenger's home before service can
be provided. Drivers are not allowed to enter homes to
assist riders. Wheelchair bound riders must be
wheelchair independent or have a caregiver to assist
them. Please be advised that this transportation program
is not an emergency ambulance or medical transport
service. Drivers are not trained medical professionals.
What does it cost?
Trips fewer than 5 miles are $3 and over 5 miles are $5,
round trip. One registered caregiver or companion is
allowed to ride with the patient at no additional
charge. Special hardship cases should be referred to the
township supervisor and may be eligible for up to 2
round trips weekly.
